This course provides a comprehensive exploration of the geriatric population outside of the acute care setting, focusing on person-centered care, social determinants of health, polypharmacy, and well-care that promotes autonomy. Students will assess the unique health, psychosocial, and systemic needs of older adults, critically examining community-based strategies to enhance quality of life, prevent readmissions, and improve health literacy.
