This course will define case management and the multifaceted roles of social workers as case managers. Attention will be paid to the ways in which social workers can identify and operationalize social work values, ethics and practice models in the context of the diverse and competing needs that are often present in case management service systems. The course will examine the importance of strengths-based and interdisciplinary teamwork to case management practice. This course also examines concepts and principles of case management practice from a trauma-informed, human rights perspective with populations and issues such as: older adults; children and families; persons with serious mental illness; substance misuse; and persons with chronic disease and/or disability. The core functions of case management practice in a range of settings are addressed in relationship to issues of diversity and empowerment.

SW 500, 503, 505, 506, 510, 514, 520, 521, 550, 551 or SW 540 and 541
